HPV forms needing to be returned

Hi Room 6. Thanks to the Year 8 students who have returned their forms already. We still need forms returned from the following people:

Owen , Maggie, Rafferty, Patrick, Sylvie

Even if you are not going to receive the immunisation your form must be returned indicating that. If you've lost your forms then please collect a new one from the school office.
